Proudly named after Dr. Frederick Banting, who discovered insulin right here at Toronto General Hospital, bant simplifies your diabetes. We have been working with patients and endocrinologists to co-design bant. The app aims to show you how your personal lifestyle impacts your blood glucose control. Through bant, you can take meal photos, track weight, capture step data, and even sync directly with your Bluetooth blood glucose meter. Over time, bant will highlight patterns and trends, enabling you to better understand how to adjust your self-care.

BrDI Flight is a computer tablet-based training prototype application able to mitigate and potentially reduce functional problems related to dementia risk.

breathe

breathe is intended to empower patients to self-manage their asthma. breathe will do this by collecting data from asthma patients on their triggers such as smoke, pollen, poor air quality, exercise etc. breathe will look at these triggers and remind patients of their asthma action plan when they might be experiencing an asthma flare-up. breathe also integrates with Environment Canada to source air quality information from across Canada and provide that information directly to the patients.

breatheAgain

breatheAgain transforms the current methods for the self-management of COPD, from a generic paper-based approach, to one that is dynamic and personalized. The mobile app enables patients to manage their exacerbations more efficiently and effectively. breatheAgain incorporates techniques used by respiratory educators and enables patients to independently identify changes in their breathing, learn techniques based on their unique medications, and highlights key activities of the patient’s COPD action plan. Together, these features support the individual in the self-management of their COPD.

CareExpress

Savience’s CareExpress is a validated software system for patient pre-registration to ER, matching with Health Information Systems (HIS) data, patient triage and streaming. This innovation will facilitate patients’ ability to triage themselves at home and in the community, as well as provide advice for alternative treatment sources in cases where attending a hospital ER may not be required.

Unfortunately, most youth do not receive the education or self-care training needed to face these challenges. iCanCope is a comprehensive pain self-management platform that caters specifically to youth living with persistent pain. Through virtual education, active goal setting features and the support of an online community, iCanCope provides an outlet for individuals to share, learn and cope.

Improving Mood in Youth

The NexJ Connected Wellness Platform will deliver interactive workbooks and related videos directly to youth. A three-arm randomized controlled trial (RCT) will be conducted consisting of First Nations-Indigenous youth (18-25 years) and youth from all other ethnic backgrounds. Specialized health coach training and related materials will support youth mental health, particularly in First Nations-Indigenous youth.

Medly is a mobile app that enables patients to complete daily symptom questionnaires and measure vital signs using Bluetooth-enabled medical devices. Based on their readings and reported symptoms, patients are provided with actionable self-care messages. If there are signals of health deterioration, clinicians receive real-time alerts with all relevant data via secured email or through the clinical user interface. Medly has sophisticated algorithms, vetted by specialists, to ensure that the alerts generated are clinically valid and relevant to reduce any alert fatigue or desensitization commonly associated with alerts.

Ned

Health Apps

Ned, short for “No Evident Disease”, is a seamless mobile platform designed for patients at all therapeutic stages of prostate cancer survivorship. It provides patients and their oncologists with a means to prospectively collect patient-reported outcomes through validated health-related quality of life questionnaires. This can be analyzed to prompt behavioural or treatment changes or to inform trends in health outcomes. Ned also promotes self-care and clinical decision making by providing patients and physicians with individual Prostate Specific Antigen (PSA) results, in real time directly from the Ontario Laboratories Information System.

OTNconnect Mobile Application

OTN aims to meet the unmet demand for an unstructured, but encrypted messaging service for ad hoc collaboration, questions and answers among clinical team members, as well as safe and secure communications between them. Preoperative Screening Patient Portal

The ForaHealthyMe Inc. solution delivers a virtual clinic without walls. The company has taken a comprehensive approach to developing solutions that address some of today’s most complex health challenges.

The platform blends clinical protocols for prehabilitation and rehabilitation patient management with real time analytics, motion tracking, 3D avatars, video and gesture recognition technologies. Combined with interactive and responsive 3D imagery to simulate and track physical activity, providers can virtually prescribe, track and analyze real-time data streams to build personalized treatment plans for patients during the preoperative and postoperative phases of care.

VITALITI

Cloud DX, Ontario’s Start-up of 2015, is developing VITALITI, a next generation vital sign monitor that can autonomously diagnose up to nineteen separate diseases. Through the use of such systems, coupled with smart software, the aim is to reduce clinical workload and optimize patient recovery. This project focused on redesigning the technology’s earpiece, otoscope, and ECG sensors to improve signal quality, reliability, performance and user experience through a human-centered design process. VITALITI was submitted to the Qualcomm Tricorder XPRIZE competition and was awarded the Bold Epic Innovator Award.
